Start of Care Registered Nurse (RN) – Home Health

Position Summary

The Start of Care (SOC) Registered Nurse is responsible for completing comprehensive initial assessments of patients entering home health services. The RN evaluates the patient’s medical needs, develops an individualized plan of care, coordinates services with the healthcare team, and ensures patients receive safe, appropriate, and quality care in the home.

Key Responsibilities

• Conduct comprehensive Start of Care assessments for new home health patients in their homes.
• Review the patient’s medical history, medications, diagnoses, current health status, and home environment.
• Assess physical, functional, psychosocial, and safety needs.
• Develop and implement an individualized plan of care based on assessment findings and physician orders.
• Coordinate nursing services and other disciplines, including physical therapy, occupational therapy, speech therapy, and home health aides, as appropriate.
• Communicate patient status, changes in condition, and care needs with physicians and other members of the healthcare team.
• Provide patient and family education regarding medications, disease management, treatments, safety, and self-care.
• Perform skilled nursing services and interventions as ordered.
• Monitor patient progress and modify the plan of care when necessary.
• Complete accurate and timely clinical documentation, assessments, and required home health records.
• Ensure care is provided in accordance with physician orders, applicable regulations, and agency policies.
• Identify changes in patient condition and determine when additional medical evaluation or intervention is needed.
• Collaborate with case managers and other healthcare professionals to promote continuity of care.
• Provide compassionate, patient-centered care while working independently in the home setting.

Qualifications

• Current and unrestricted Registered Nurse (RN) license in the applicable state.
• Previous home health experience strongly preferred.
• Experience completing Start of Care assessments preferred.
• Strong clinical assessment, communication, organization, and documentation skills.
• Ability to work independently and manage a caseload effectively.
• Reliable transportation and ability to travel to patients’ homes as required.

Preferred Skills

• Knowledge of Medicare/home health documentation requirements.
• Familiarity with electronic medical records (EMR) systems.
• Strong time-management and critical-thinking skills.
• Ability to establish positive relationships with patients, families, physicians, and healthcare teams.
• Commitment to providing safe, high-quality care in a home-based environment.
